Job Title:Case Manager – Vocational & Work Services
To provide quality vocational rehabilitation, vocational skills instruction, job training, support services and case management to individuals with disabilities placed in work services, supported work, supported employment, and competitive employment. To provide supports to individuals which accommodate the individual’s work schedule and hours.
1. Collects, organizes, and analyzes information about individuals through records, tests, interviews, and professional sources, to appraise their interests, aptitudes, abilities, and personality characteristics, for vocational and educational planning.
2. Compiles and studies occupational, educational, and economic information to aid individuals in making and carrying out vocational and educational objectives. This includes the development and coordination of the implementation of Individual Rehabilitation Plans. (I.R.P.) and the Work Evaluation Plan.
3. Schedules individuals’ vocational services and supports, and refers individuals to placement service where appropriate, facilitates referrals to alternative agencies/programs for ancillary services as identified by the individual or plan, i.e. personal psychological therapy, speech therapy, mobility training, etc.
4. Assists individuals to understand and overcome social and emotional problems.
5. Engages in research and follow-up activities to evaluate service and support techniques.
6. Participate in the referral/acceptance process of new clients, including interview of referrals.
7. Provide program orientation to individuals, including Human Rights training.
8. Train and assist assigned individuals on site for competitive and supported employment jobs, individualizing training methods according to job requirements and individual needs and support them to adapt to employment sites, work demands and expectations through supportive services and orientation.
9. Provide job coaching supports which assist individuals develop appropriate coworker relationships on the job and develop a natural supports system. Assist individuals in developing transportation arrangements, including training in use of public transportation. Conduct written job analysis of on-site assignments to be utilized in training and assessment. Serve as an advocate for the individual at the work site. Recognize, assess and resolve problems/situations at the job site. Maintain open communication and positive rapport with host employer and employees.
10. Maintain permanent client case records including comprehensive progress notes and updated client data according to policy and requirements.
11. Maintain on going communication and support with work supervisors and other pertinent staff regarding client issues/services.
12. Participate as a member of the Rehabilitation Team. Participate in the ongoing assessment, modification, and development of vocational programs and services.
13. Represent the agency with external personnel.
14. Perform other duties deemed necessary.
Qualifications: Bachelor's degree (B. A.) from a four-year college or university; and one to two years related experience and/or training; or equivalent combination of education and experience
Salary: This is a full time position with benefits and a salary range of $26,000 - $30,000 commensurate with education and experience.

Job Title:Case Manager – Adult Foster Care
Implements the Policies and Procedures of Community Supports to assist participants in establishing program goals and positive outcomes
Interviews participants and care providers for the Community Support services with empathy to problems such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem and recommends to the Community Support team admission and individual goals.
1. Secures information such as medical, psychological, and social factors contributing to participant's situation, and evaluates these and participant's capacities.
2. Counsels and trains participant(s) individually, in family, or in other small groups regarding plans for meeting needs, and aids participant to mobilize inner capacities and environmental resources to improve social functioning.
3. Completes required home visits to participants within the Community Support services.
4. Helps participant to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and participant's part in creating them.
5. Refers participants to community resources and other organizations, as needed.
6. Compiles and maintains participant records as required.
7. Prepares required reports to insure the Community Supports meet all requirements and regulation and inform supervisor and agency on any conditions in participant situations.
8. Reviews service plan and performs follow-up to determine quantity and quality of service provided participant and status of participant's to meet personal goals and service objectives.
9. Accesses and records participant and community resource information.
10. Secures supplementary information such as employment, medical records, or school reports.
11. Assists with the determination of a participant's or care provider’s eligibility to participate with the Community Support services.
Qualifications: Bachelor's degree (B. A.) from a four-year college or university; and one to two years related experience required.
Salary: This is a part time position – 30 hours with an hourly range of $13.00 - $14.75 commensurate with education and experience.
