Objective: The AFC provider must employ a care manager to provide psychosocial and counseling assistance for the AFC program. This position is an intergal part of a multidisiplinary team that works hard. Education and Experience:Bachelors Degree, social workers license from the Massachusetts Board of Registration in social work. At least two years or recent experience working with eldery or diabled population. Certificates, Licenses, Registrations, Personnel Requirements: Valid Driver's License, recent physical, T.B. screening within the past 12 months, and Bachelors Degree.

Job Title: Healthcare Supervisor
Department: Day Habilitation
Objective: Oversee nursing and medical services offered through the Day Habilitation Center
Education and Experience: Must hold a registered nurse license and one to two years experience in a community health setting.
