Adult Foster Care benefits the following people:

Families who need some additional supports such as nursing, social work, respite, and a financial stipend to help keep their relative at home.

Caregivers who open their homes to provide guidance and supervision to AFC members. The families have a desire to help others, and welcome a member into their families.  In return the families gain personal fulfillment by caring and providing help to a person in need.  In addition they receive a monthly stipend from MassHealth.

 AFC members are adults unable/willing to live independently because of the challenges of aging and/or disabilities. Members enjoy social connection living with a family brings, while receiving assistance with daily living skills and personal care activities.

You are eligible to become a member if you…

 Are a recipient of MassHealth.

 Are 16 years of age or older.

 Want to live in a home environment and participate in family activities.

 Have a medical, mental or developmental condition that requires daily physical assistance OR cueing and supervision during at least one of the following activities: bathing, dressing, eating, transferring, ambulating, or toileting.

 Have or want to have a productive way to spend your week days.

 Do not abuse alcohol or drugs.

 Are in need of psychiatric or behavioral support.

 Be willing to review your services monthly with the AFC Care Manager or Registered Nurse.

You can be an AFC Caregiver/ Host Family if you…

 Will be able to devote appropriate time necessary to provide needed supports and personal care to the AFC Member.

 A family member who is not the guardian

 Are a responsible person who is at least 18 years of age, with the ability to make mature judgment.

 Do not abuse alcohol or drugs.

 Can ensure the member's safety and well-being at all times.

 Provide a home that meet all safety and AFC Regulations, i.e. in good repair, fire extinguishers, first aid kits, fire and CO detectors, and appropriate unobstructed exits.

 Have a clean Criminal Offender Records Information (CORI).

 Have a physical examination and TB test within the past 12 months

 Are able to pass the 8 hour training requirement
